there is only one file in your sample. Remove all unnecessary files styles,images,dll and then attach it again. The best would be if you manage to make an isolated sample. I tried to create sample based on the other attached file but the issue doesn't exist. 

Attached is my actual code which is similar to my sample, but the calendar is not displaying.

Hello!  I created a sample, however the calendar appeared (see testapplication.zip).  My actual project is still not displaying the calendar which I both use the same code (see LicenseeApplicationDetail.zip (next upload) )
