Tools in the toolbar don't always load

Hi,

I have about 15 tools loaded on the toolbar and sometimes they dont all load. I create multiple instances of the toolbar as multiple windows are opened, but sometimes only some of the toolbar loads, the rest doesn't.

Please advise.
