This started after the update to 2083.
If you group by multiple columns, the first column is not correctly grouped.
I have also attached a program stub where you can quickly see this in action.
If you group by Therapist and then by Visit Clinic you will see that the 'therapist' become ungrouped an you end up with individual groups.

You assume incorrectly. The fix for this issue is in the 10.2 release. :)
